By checking this box, you agree to the terms outlined, and provide consent for your child(ren) to attend Emerald Crossing Vacation Bible School (VBS) and participate in all activities.
Medical Release:
I understand that no child(ren) can attend VBS if they are ill. If my child(ren) becomes ill while at VBS, I will be called to pick him/her up. I am aware of VBS activities and that there can be slight risk to injury as well as close contact with others being a risk for contracting illness. I willingly agree to assume responsibility for those risks as a condition of registering my child(ren). I understand that if my child(ren) does not follow the rules set by VBS, Grace Baptist Church and it's volunteers/staff, and endangers his/her own or another’s safety and/or health, my child(ren) will be asked to leave the program and may not be able to return.
Church Release:
I hereby release Grace Baptist Church of Papillion, its staff, volunteers, and representatives from any liability for injury or illness suffered by the registered child(ren) and agree to release indemnify and waive any rights by subrogation I may have, and hold harmless Grace Baptist Church of Papillion, its staff, volunteers, and representatives for claims or asserted injury, illness or damage to my child(ren).
